Your rights and how we look after your information

The Avon Partnership NHS Occupational Health Service (APOHS) is a partnership between the University Hospitals Bristol NHS Foundation Trust, North Bristol NHS Trust, and Weston Area Health Authority. APOHS provides occupational health advice and services to the partners and their employees and to other customer organisations and their employees.

The partners are joint Data Controllers of personal data of users of APOHS’ services for the purposes of the Data Protection Act 2018 and the General Data Protection Regulation 2018 (GDPR). Likewise, other APOHS Customer organisations are Data Controllers for data regarding their staff. As Data Processor, APOHS takes your confidentiality and privacy rights very seriously. This notice explains in detail how we collect, process, transfer and store your personal information and forms part of our accountability and transparency to you under the GDPR.
